Does anyone wear a helmet?

I only wear my helmet on our sea cave trips to Santa Cruz Island.  If you go in the caves, you have to use one.  It would be a very good idea to wear one in the surf, I should, but I never bother.

Wouldn't surf with a helmet with a fixed peak.....might cause a bit of neck issues if you wipeout at speed and hit the water wrong.....

That's a good point.  I don't think it is a problem with the Gath, you can dive with it and because it is molded close and relatively thin to the head, it does not pick up undue force from the waves.  I don't think the brim types or thicker types would fare so well with neck twisting in strong water.
