Believe it or not Phoenix, AZ.....Pizzeria Bianco.  Been there a few times, each time the wait was at least 3 hours and worth it.  Pizzeria Mozza in Newport Beach/LA is a close second.

Bizzaro Pizza in Melbourne Beach, FL.  Great NY style pizza and Calzones.

For deep dish: Gino's East in Chicago, The Meaty Legend. Look no further.

Pizza Port in So Cal various locations is a pretty good pie.

Partial to Deep Dish from Zachary's Pizza in Berkeley, San Ramon and Oak-town. 
Not a thin crust fan.
Postscript: there's a lot of bad pizza out there, way more than there should be.

I tend to agree.  I grew up in NJ and maintain that you can pick any seedy joint in NJ and bet good money that the pizza there will be better than anything you can find anywhere else.  If you walk into a place and the proprietor swears in Italian and asks "what the hell do YOU want?" order with confidence and be ready for a treat.

That said, I did eat at Regina's in Boston this summer and it was outstanding.

Escape From New York in Portland makes a good straightforward pie. It's very tough to beat the pizza in any small town in Italy though. Keep it simple, Margherita (tomato sauce, anchovies and cheese). Transcendental.
Foote 10'4X34", SIC 17.5 V1 hollow and an EPS one in Hood River. Foote 9'0" x 31", L41 8'8", 18' Speedboard, etc. etc.

That's my favorite here also--they keep it simple.  I'm not a fan of the goat cheese/apples/nuts/etc. that other "highly-rated" places here have.  Best test to me of good pizza is getting a plain cheese slice. 

Other than that, the best pizza is free pizza.

My vote is for Patsy Grimaldi's under the Brooklyn Bridge in good ol' Brooklyn.

Also Mimi's on Lexington and 83rd. God, I'm salivating just thinking about it!
